Specifying the Patient's Home Address

1.  In the ADDRESS field, specify the address of the patient’s home.

2.  Type to search for the address you are looking for. Be sure to enter the entire address including city and state, then select the correct address from the list that is generated.

3.  When you select an address, the map below will display the corresponding location. You can move the red pin on the map to specify the exact coordinates that will be passed on the driver. For example, if you want the driver to navigate to the corner of the lot when they arrive at this location, you can move the pin to that exact corner.

4.  If applicable, enter an apartment or building number in the APARTMENT NUMBER field.

5.  If there is important information that needs to be conveyed to drivers when they pick the patient up at their home, enter this information into the PICKUP NOTES field. For example, “Please wait for Darius outside of apartment A1.”

6.  If there is important information that needs to be conveyed to drivers when they drop the patient off at their home, enter this information into the DROP OFF NOTES field. For example, “Please drop Darius off in front of apartment A1.”

7.  Click Continue.
