On Dashboard View navigate to green “New Trip Request” located in the upper left-hand corner. Click to generate the Trip Request Form.
In the Patient Information section, “Who is the Trip for?”, the User can type to search for and select existing patients. IF a Patient does not exist the User can select “Create new patient” after typing in the patient’s name [See How to Create a Patient].
The patient’s information populates based on their saved profile. This information is editable if this needs to be edited in the Trip Request Form at the ride level. Patient Information includes demographic details, such as First name, Last Name, Language, Phone number, & Date of Birth.
Below the Patient Information section are the Request Details. The User must select the Reason for Request from a dropdown menu, options are Appointment, Discharge, or Other.
The “Reason” for a request will auto-populate the remainder of the Request Details. Selecting “appointment” defaults to a round-trip ride where ride #1 is scheduled and ride #2 is on-demand. Selecting discharge defaults to a one-way ride where the ride is on-demand. Selecting other defaults to a round-trip ride where ride #1 is scheduled and ride #2 is on-demand.
Once the reason for the request is selected, the User must specify the date the ride(s) is needed. The appointment date defaults to the current date, while a calendar icon appears to allow the correct date and time to be selected by double-clicking on the date.
The Pickup Location is for an appointment ride. The Apt Number/Entrance field allows the user to record more specific information about the pickup location such as apartment, unit, or building number. This information is then transmitted to the driver. [See Location]
Pickup Time will populate if Reason for Request is an “appointment” or “other” trip type. [See Pickup Time].
Estimated Pickup Time only populates when the reason for the request is an “appointment” or “other”, and the estimated return time defaults to on-demand and this field specify the time ride #2 is expected to be needed. It defaults to an hour after the appointment time but is editable.
Upon the completion of the Request Details, the User will continue to the Trip Summary page. This page displays all the rides in the request which can be viewed by scrolling down the page. The User is able to view, edit and add detail for each ride, by selecting the pencil icon located in the upper right-hand corner of Location, Point of Contact, and Mobility. By default, each ride in the request is comprised of the same location, point of contact, and mobility information. However, the parameters of each ride can be customized.
An “Update other rides” banner will appear when a User has made an update to a portion of a patient’s ride to verify that the update should be applied to all rides in the request. Selecting “Yes” will apply all changes to the current ride to the other rides in the trip request. A User can identify a patient who would only need an update for one ride by selecting “No” and not applying the updates to other rides.
The Locations section reflects the information the User entered on the Request Details Page. When a User selects the pencil icon located on the upper right-hand corner of the Locations Tab the information becomes editable. [See Locations].
The Communication Method for the Point of Contact will determine how the patient and/or proxy are notified about the details about their trip. The patient ’s information will automatically populate as the point of contact. [See Point of Contact]
The Mobility section reflects how the patient ambulates and therefore the type of vehicle and level of assistance that they require. The User can select the pencil icon on the Mobility Tab’s upper right-hand corner. [See Mobility]
Users have the option to request a Recurring Ride for patients who receive repeat appointments. Once a User selects “Yes” the ride does repeat, they are able to identify the days of the week the ride repeats. The User selects the days of the week the ride repeats and the date upon which the Trip sequence ends. [See Recurring Requests]
Once a User has completed the Trip Summary they must select “Submit”, for the submission of the ride request.
Once a User has submitted a Trip for a patient, it will appear on the Dashboard View and allow the User to view the real-time Ride Status.
